Benefits-eligible faculty and staff who did not change health plans for 2010 should continue to use 2009 insurance cards.
New cards will be mailed only to employees who enrolled for the first time or changed health plans.
For prescription ID cards, only the PPO plan issues a separate card. All other health plans use the health ID card for prescriptions. The PPO plan has a health card group number BOR018000 and a Medco pharmacy card. The HMO plan issues a single health card with the group number 1000012180. The high deductible PPO plan has a single health card with the group number BHD0180001, and participants in the plan should have received a welcome kit from US Bank.
