To be effective in the event of an emergency, UGA officials want to be able to identify and support students, faculty, staff and visitors with a disability who would need assistance.
Individuals with a disability, even if those who have not otherwise self-identified or asked for an accommodation, are asked to complete an Emergency Assistance Referral Form if they feel they would need assistance during an emergency.
Completed forms will be kept on file by the Office of Emergency Preparedness and will be used only to develop an emergency plan for individuals who want one.
