By Jan. 31, UGA employees (and their spouses or dependents) must disclose all business transactions during the preceding calendar year between the state of Georgia and the employee.
The form for reporting business transactions consists of two pages and is available on the Web at www.usg.edu/admin/legal/documents/bus_transact.pdf. The first page calls for identifying information and the second page is where the transactions are actually listed.
Copies of the form are also available from the Office of Legal Affairs (542‑0006).
If the amount of each individual transaction was less than $250 and the aggregate of all transactions did not exceed $9,000, no report need be filed.
